Leaky Gut Symptoms: What the Science Actually Supports

Search leaky gut symptoms and you will find lists claiming that intestinal permeability causes everything from bloating to depression to acne. Some of that has genuine research behind it. A good deal of it does not, and the gap between the two is where people lose money and time. We want to be straight about where that line falls, because we work with gut health every day and we think the overclaiming does real harm to a real subject. Increased intestinal permeability is a documented, measurable phenomenon that appears in a number of well-characterised conditions. What it is not, on current evidence, is a proven single cause of a long list of unrelated complaints.

What intestinal permeability actually is

The lining of your small intestine is a single layer of cells. That is the whole barrier between the contents of your gut and your bloodstream: one cell thick, across a surface area far larger than most people imagine.

Those cells are joined by protein complexes called tight junctions. Tight junctions are not permanent seals. They open and close under regulation, because the barrier's job is selective: nutrients through, most everything else not.

Increased intestinal permeability means that regulation has shifted and the barrier is letting through more than it should. This is the actual, non-controversial science. It is measurable in a laboratory, it is documented in the literature, and it is not in dispute.

The term leaky gut is the popular name for it. The scientific literature generally says increased intestinal permeability, or refers to intestinal barrier dysfunction. Same underlying phenomenon, considerably more careful framing.

Where the evidence is strong

Increased permeability is well documented in coeliac disease, where gluten exposure in genetically susceptible people drives both barrier disruption and immune activation.

It is documented in inflammatory bowel disease (Crohn's disease and ulcerative colitis), where barrier dysfunction is an established part of the picture, and where it has been observed in some first-degree relatives of patients too.

It appears with nonsteroidal anti-inflammatory drug use. NSAIDs affect the intestinal barrier, and this is one of the better-characterised drug effects on permeability.

It is documented in significant alcohol consumption, in serious burns, in major trauma and in critical illness, where barrier failure is a recognised clinical concern.

And there is a growing body of work on permeability in metabolic conditions and in a subset of irritable bowel syndrome. That research is genuinely active and genuinely interesting, and it is mostly about association rather than settled causation, which is a distinction worth holding onto.

Where the claims outrun the evidence

Here is the part that gets left out of most articles on this topic.

In several of the conditions above, it is not established whether increased permeability is a cause of the condition or a consequence of it. The barrier can be damaged by inflammation as readily as it can contribute to it. Observing both together does not settle which came first, and in a number of cases the honest answer is that we do not yet know.

The long symptom lists are where things get loosest. Claims that intestinal permeability causes depression, anxiety, acne, joint pain, chronic fatigue, brain fog and autoimmunity are typically extrapolated from mechanistic plausibility and small studies, not from evidence that treating permeability resolves those complaints.

The mechanism is plausible. Plausible is not the same as demonstrated, and a symptom list assembled from plausibility will match almost everybody, which is precisely what makes it useless as a diagnostic.

There is also no established consensus that a leaky gut protocol reliably reverses permeability or improves outcomes. Some interventions have supporting evidence for specific situations. Most supplement stacks sold for this purpose do not.

Symptoms that genuinely warrant a look at gut function

None of this means digestive symptoms should be dismissed. It means they should be investigated properly rather than attributed to one fashionable mechanism.

Persistent bloating, particularly with a clear relationship to meals. Changes in bowel habit that have lasted weeks rather than days. Reactions to a widening range of foods. Reflux that is not settling. Ongoing abdominal discomfort.

These are worth investigating, and they are worth investigating with tests that measure something (what is living in the gut, how well it is digesting and absorbing, whether there is inflammation present) rather than with a symptom questionnaire that leads to a predetermined conclusion.

And some symptoms need a physician before anything else. Blood in the stool. Unexplained weight loss. Difficulty swallowing. Persistent vomiting. Anaemia. Fever alongside gut symptoms. A significant change in bowel habit in anyone over fifty. A family history of inflammatory bowel disease or bowel cancer. These are not functional testing questions and we refer them on.

How permeability is measured

Because this is where a lot of the confusion sits.

The research standard is a differential sugar absorption test. You drink a measured solution of two sugars, typically lactulose and mannitol, and their ratio in urine over a set window indicates how much of the larger molecule crossed a barrier it should not readily cross. It is the most established approach and it is mostly used in research settings.

Blood zonulin is offered widely in functional testing. Zonulin is a protein involved in tight junction regulation, and it is the marker most often cited. It is worth knowing that there is real scientific debate about what commercially available zonulin assays actually bind to, and how well the results correspond to permeability measured by other means. It is a signal, not a verdict.

Stool zonulin appears as an add-on to panels including the GI-MAP. The same caveats apply, with the further point that it is one marker on a panel and gains most of its meaning from what sits around it.

Antibody panels marketed as leaky gut tests vary considerably in what they measure and how well validated they are. Ask what specifically is being measured and against what reference before paying for one.

A more useful question than do I have leaky gut

In practice, we find the permeability question is rarely the most productive place to start, not because it is unreal, but because it is downstream.

The more useful questions are what is actually living in your digestive tract, whether you are digesting and absorbing what you eat, whether there is measurable inflammation, and what in your history plausibly set the current state up.

Those have concrete answers. A stool panel measures organisms, digestive markers and inflammatory markers. Bloodwork read against functional ranges shows nutrient status and patterns. Your history usually explains more than any single test.

Where permeability is part of the picture, it tends to show up inside that broader assessment rather than needing to be chased on its own. And where the underlying drivers are addressed, the barrier is generally in a better position to do its job.

Is leaky gut a real medical diagnosis?
Increased intestinal permeability is a real, measurable phenomenon documented in conditions including coeliac disease, inflammatory bowel disease and NSAID use. Leaky gut syndrome as a standalone diagnosis explaining a broad list of unrelated symptoms is not a recognised medical diagnosis, and the distinction between those two statements matters.
What test shows leaky gut symptoms are caused by permeability?
No test establishes that. The differential sugar absorption test measures permeability itself and is the research standard. Zonulin markers in blood or stool are widely offered but debated in terms of what they measure. None of these demonstrate that a given symptom is caused by permeability, which is why the finding has to be read alongside history and other labs.
Can leaky gut be reversed?
Barrier function can improve when what is driving the disruption is addressed, for example by removing an NSAID, treating coeliac disease with a gluten-free diet, or managing an underlying inflammatory condition. There is no established consensus that a generic supplement protocol reverses permeability, and claims to that effect run ahead of the evidence.
Should I do an elimination diet for leaky gut symptoms?
Not as a first move, and not indefinitely. Elimination diets are a useful short-term diagnostic when structured and time-limited, but long unsupervised restriction narrows the diet, reduces microbial diversity and can create its own problems. If you are eliminating foods and the list keeps growing, that is a sign to get an actual assessment rather than to cut further.
